
Onier Villarreal, MD, MBA, FACEP
Chief Medical Officer & Board-Certified Emergency Medical Physician
Dr. Onier Villarreal was born and raised in Puerto Rico. He holds a degree in mechanical engineering from the Massachusetts Institute of Technology (MIT). He completed his medical training at the University of Puerto Rico and his post-graduate fellowship in trauma and surgical critical care at the R Adams Cowley Shock Trauma Center, affiliated with the University of Maryland in Baltimore, MD. Additionally, he earned an MBA from Johns Hopkins University. He is board-certified in Emergency Medicine, Critical Care, and Neurocritical Care.
While remaining clinically active, Dr. Villarreal pursued a career as a physician executive. He served as the Medical Director and Executive Vice President for Envision-HCA’s West Florida Division. Since 2017, he has been the Chief Medical Officer and Executive Vice President for Southeast Emergency Partners, a company he founded that serves Hernando and Citrus counties in Florida. He has also held the position of Medical Director for the ICU at the TGH North Crystal River location.
In 2018, Dr. Villarreal began developing his most recent project, Starkey Ranch ER & Hospital, which opened in December 2024. He serves as the Chief Medical Officer and one of the Physician Owners of the facility.
Currently, Dr. Villarreal is the commander and flight surgeon for the 927th Aerospace Medicine Squadron at MacDill Air Force Base, where he holds the rank of lieutenant colonel. In his free time, he enjoys a good bottle of wine, hiking, and spending time with his amazing wife and four children.
